Below you will find the application form for one-off compensation from the Fund. It is important that you fill in the form as completely as possible. If you need help, you can contact an office of Victim Support Netherlands [Slachtofferhulp Nederland] in your area. The addresses of its branches are listed in the Yellow Pages [Gouden Gids]. 2. Application form for compensation by surviving relatives and/or partners 

If you are a surviving relative or partner of the deceased victim of a violent crime, you can use the “survivors’ compensation application form” to apply for compensation from the Fund.
